Carolinas AGC announced SCDOT has issued new insurance requirements for contractors doing work for SCDOT. The revised certificate of insurance form is found Here that contains a sample ACORD 25 form with required information, minimum policy limits, and other required notations. A Liberty Mutual form may be used, but it must have the same required information, minimum policy limits, and other required notations as in the sample ACORD form.
You will also find, in the link a Contractors Insurance Checklist, which distinguishes between mandatory and optional requirements. This Checklist must be completed in its entirety and submitted with the Certificate of Insurance.
